1. Pesticide Poisoning, Multan, Pakistan (2021)

Farmers on local cotton farms were exposed to toxic pesticides during spraying without any protective equipment, resulting in 5 deaths and over 20 hospitalizations. Root causes: no gloves or masks provided, no training on chemical handling, and improper handling practices generally. Fixes: mandatory PPE for all spraying work, structured chemical safety training, and awareness of Material Safety Data Sheets (MSDS) for the chemicals in use.

2. Tractor Rollover Fatality, Faisalabad, Pakistan (2020)

A farmer was electrocuted by exposed wiring near an irrigation tube well, resulting in one fatality. Fixes: proper earthing of all irrigation electrical systems, regular inspection, and electrical isolation procedures before maintenance.

4. Grain Silo Suffocation, Sindh, Pakistan (2022)

A worker entered a grain silo and suffocated due to low oxygen levels, resulting in one death. Root causes: no confined space entry procedure, no gas testing before entry, and no rescue plan in place. Fixes: a formal confined space permit system, mandatory gas monitoring before and during entry, and a standby rescue team for any confined space work.

A worker’s hand was trapped in a harvesting machine during operation, resulting in amputation. Root causes: no machine guarding in place, no lockout/tagout (LOTO) procedure followed, and unsafe maintenance practices.
